MEMBERSHIP PLANS REQUIRE AN INITIAL 3-MONTH COMMITMENT AND CONTINUE MONTH-TO-MONTH THEREAFTER UNTIL CANCELED.
A ONE-TIME MEMBERSHIP SETUP FEE APPLIES TO PROPERTY ONBOARDING, OPERATIONAL SETUP, ACCESS COORDINATION, AND ACCOUNT PREPARATION. THIS FEE IS CURRENTLY WAIVED FOR FIRST-TIME EVHS MEMBERSHIP ENROLLMENTS. MEMBERSHIPS CANCELED AND LATER REACTIVATED WILL REQUIRE A NEW SETUP FEE AND MAY BE SUBJECT TO CURRENT PLAN PRICING.
